Clinical Evaluation of Cefmetazone in Surgical Patients

Abstract
|
|
|
The use of appropriate antibiotic in surgical patients is very important perioperatively and the ideal antibiotic should be sensitive to wide anti-microbial spectrum, be minimal side effects and low cost.
Cefmetazone (CMZ) which is a semisynthetic second generation cephamycin derivative, has wide antimicrobial spectrum and strong resistance to beta-lactamase.
To evaluate its efficacy and safety, laboratory and clinical studies were performed in 25 surgical patients between January and June, 1986 at KangNam St. Mary¢¥s Hospital.
The drug was administered 2.0 to 3.0gm divided in 2 or 3 doses by intravenous dripping and authors checked the effective rate of CMZ, CMZ-sensitivity test by disk dilution method and side effects.
The clinical results were as follows:
1. CMZ was effective in all 7 cases (100%) of the prophylactic use group consist of biliary diseae and the effective rate of therapentic use group was 88.9% (16/18): 87.5% (7/8) in perforated appendicitis, 100% (4/4) in duodenal ulcer perforation, 83.3% (5/6) in abscess on various site and the overall clinical efficacy rate was 92%.
2. In CMZ sensitivity test, CMZ was very sensitive to Krebsiella group (100%), Proteus group (100%), E. coli (91.4%),Serratia group (80%) but lesser sensistive to Pseudomonas group (20%),Enterobacter group (20%) and also very sensistive to anerobic group (81.8%).
3. The side effects of CMZ was found in 4 cases: 1 case of cutaneous hypersensitivity, 2 cases of elevated serum SGOT and SGPT, and one case of elevated serum BUN and creatinine.
